Failed to install the Extension Pack D:/mwarner/Downloads/Oracle_VM_VirtualBox_Extension_Pack-5.1.18-114002.vbox-extpack.

Do you have an antivirus running? Try disabling it temporarily. They have been known to interfere with the installation of the ExtPack. My gut feeling is that an "unknown" application (VirtualBox) is trying to modify a folder in the "Program Files" and they are over-jealous about protecting it, going a step further than even Windows itself.

Correct! On my system it was also the virus scanner. I have disabled it. After that I could successfully install the package "Oracle_VM_VirtualBox_Extension_Pack-5.1.18-114002". My OS is Windows 10 Pro x64.

Great, I guess the ticket can now be closed.
May I suggest something? It's usually better and faster, if problems like this one (configuration, question) get first addressed in the forums (https://forums.virtualbox.org/). More than 95% of the issues are resolved over there, which keeps the developers focusing on the bug fixes and enhancements, and there is no need for another ticket to keep track of. For example, yours is not a bug and someone from the developers has to deal with it and close it as "Invalid".

not sure why this keeps happening for the last few extension packs/vbox versions
